Premiums, eligibility, effective dates, underwriting, waiting periods, exclusions and benefit limits depend on the actual policy and application. Only the applicable insurer can approve coverage and determine claim payment.
Agency and insurer roles
HQINS is High Quality Insurance Solutions, a DBA of CR Financial and Associates, Inc., California Insurance License No. 0G53185. Services are offered only where appropriately licensed. The carrier named in your contract is responsible for the insurance benefits.
Aflac individual and group products can involve American Family Life Assurance Company of Columbus, American Family Life Assurance Company of New York, Continental American Insurance Company, Tier One Insurance Company or other entities identified in the product materials. Use the insurer name on your own policy.
Different products, different purposes
Supplemental insurance is not major medical insurance. Dental and vision plans, life insurance, Medicare Supplement, pet insurance and administrative services have distinct purposes and terms. Do not assume that a feature of one product applies to another.
Pet coverage is offered through Trupanion and underwritten by American Pet Insurance Company. Aflac is not the pet insurance underwriter. Medicare Supplement is distinct from Medicare Advantage and is not connected with or endorsed by the U.S. government or the federal Medicare program.

Scenarios are fictional, and calculator inputs are assumptions selected by the visitor. Results are educational arithmetic, not quotes, personalized advice, claim predictions or coverage recommendations. No testimonials or guaranteed outcomes are implied.
